Adult Patients 18+ Years or Older

Vermont Residents Only

Eligibility for Medical and Diagnostic Lab Financial Assistance Program:

Uninsured Patients – Individuals without health insurance who struggle to access necessary medical care.

Underserved Patients – Those facing challenges beyond financial hardship, such as provider shortages, transportation difficulties, or other barriers to care.

Have a household income 400% or below the federal poverty level.
